You can request an Adult Therapy Session or Pediatric Medication Evaluation via email or phone. Please complete and return the prospective client form that will be sent to you and or staff will contact you to schedule if we have availability. Please note, due to high demand, we are not able to return calls to all prospective clients. Completing this form does not establish provider-client relationship.

First Appointments
LWC utilizes Simple Practice electronic medical record. The new client packet will be sent to you via the Simple Practice client portal. Please complete this at least 48 hours prior to your session/initial evaluation or the appointment will have to be rescheduled.
